Prepay Base Allowance

The Prepay Base Allowance is the amount you would like to spend on food and/or beverage for this reservation. A 20% gratuity and local state sales tax will be applied on top of your Prepay Base Allowance. These fees will be outlined in the shopping cart and at checkout to provide you with an estimated total charge but your card will not be charged the final amount until after the reservation occurs.* If you would like to change the gratuity amount, please call the restaurant after completing this order.

Terms of Service *

The actual amount charged to your credit card will be different than your Prepay Base Allowance but will not exceed the total authorized amount of Base Allowance plus gratuity and sales tax. After goods and services have been delivered on the date of the reservation, we will charge your card for the final amount spent on the following business day. That final amount will include the cost of ordered items plus a 20% gratuity and local state sales tax. Should the designated amount not cover the entire bill, the guest will be responsible for covering the remaining balance.

You must call the restaurant after filling out this form to confirm receipt. This is especially sensitive for those submitted within 24 hours of the reservation time. This form must be submitted before the reservation start time.

*Although your card will not be charged until after the reservation date, a pending authorization will appear on your credit card statement in its place. If you use a debit card, your bank may withhold this money from your account until the final transaction is complete.

GIFT CARDS
Gift cards are store value cards and can be redeemed at any of our three locations any time we are opened for goods, services, and gratuities. The Bohlsen Restaurant Group includes Prime: An American Kitchen & Bar, Tellers: An American Chophouse, and H2O: Seafood & Sushi. Gift Cards cannot be used on Central Market by BRG orders. Gift Cards cannot be replaced if lost, stolen or destroyed unless previously registered. Gift Cards that have been previously registered can be replaced by calling our main office at 631.277.5400 during normal business hours (M-F 9-5) and then can only be re-issued for the amount that is remaining on the card at the time of the request. Shipping and handling charges will apply.
COMPLIMENTARY CARDS
Complimentary Cards are issued free of charge as a courtesy, a promotion, or a donation. Complimentary Cards can be redeemed for goods and services, but not for tax & gratuity. You will be taxed on the complimentary portion of your bill. Complimentary Cards cannot be replaced if lost, stolen or destroyed unless previously registered. Complimentary Cards that have been previously registered can be replaced by calling our main office at 631.277.5400 during normal business hours (M-F 9-5) and then can only be re-issued for the amount that is remaining on the card at the time of the request. Shipping and handling charges will apply. Complimentary Cards that are issued for promotional purposes cannot be replaced. Cards issued for promotional purposes are packed with the specific rules & regulations that govern its use. The card is only valid for the promotion the card is associated with only during the promotional period. Complimentary cards can only be redeemed at the restaurant which issued the card. Cards issued for all other reasons other than for promotional purposes expire six months after date of issue. Complimentary Cards cannot be combined. One per check/table.
